Overview:

We strive to empower members who have behavioral disabilities to achieve their day-to-day goals and to help them gain access to available community resources, in a safe and enjoyable manner.

Function:
Direct Support Professional (DSP) provides personal support care and related services to individuals in their home. Serenity Residential Care is seeking qualified individuals interested in caring for and supporting individuals with cognitive, intellectual, psychological, and physical disabilities. DSP will be a daily living coach to client with instruction and supervision by House Manager. DSP will develop a relationship with clients, coach & assist them through the shift, and report all relevant communication to House Manager.

Responsibilities:
- Provides direct care to those with intellectual and developmental disabilities based on individual plan of support. Ensures privacy, satisfaction, and dignity in support provided
- Helps member to maintain personal hygiene and assist in ensuring a healthy, safe environment
- Plans and prepares nutritious meals
- Fosters positive relationships between caretakers and individuals served
- Assists the member with ambulation as approved
- Encourages member to become independent and coaches on daily life skills
- Attempts to promote member’s mental alertness through involvement in activities
- Gives emotional support to member and clear communication to member’s family
- Observes & reports any change in the member’s mental or physical condition
- Performs routine housekeeping tasks as relates to a safe and comfortable environment
- Prepares visit reports and incorporates into client weekly record
- Works with personnel of community agencies involved in the member’s care
- Meets regularly with supervisor about member’s status and care, and participates in planning
- Promptly reports any change in member’s condition to supervisor
- Support occurs in the home and may include providing personal care, assistance with daily living activities, teaching/developing skills, assisting in communication and self-expression, and ultimately supporting the individual in meeting his or her own goals
- Ability to drive and assist members in and out of a vehicle
- Attend in-service training as required by law
- Performs relevant duties as needed

Workplace:

DSPs work primarily in a home of the individual in a Serenity Residential Care program. Work environment may be fast paced with interruptions, distractions and deadlines. DSPs need to be flexible, adaptable, creative, and communication clearly to adjust to changing needs or activities of member. Depending on the needs of member, work can be physically demanding including transferring or supporting people with mobility needs, lifting or moving items up to 50 lbs, walking or standing for extended periods of time or providing support through CPR/First Aid, behavioral support, and other interventions.
With reasonable accommodation, hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the member’s condition and to respond according to law, regulation, policy and or practice. On occasion, may be required to bend, stoop, reach and move member weighing up to 250lbs. Must be able to communicate well and clearly both orally and in writing

Qualifications:
- High school diploma or GED
- Ability to pass criminal / registry background checks
- Clear Tuberculosis screening
- A valid driver’s license & insured vehicle
- Ability to pass and maintain required trainings such as CPR/First Aid, CRMA, DSP/CDS, and CPI
- Experience working with individuals with autism and other developmental disabilities (3-5 years preferred)

DSP II: Includes all above and completes all required trainings and coaches staff member engagement

DSP III: Includes all above and has 3 to 5 years of industry experience

Function: House Manager is responsible for operations one of SRC’s in home care residential program. House Manager ensures quality services provide to client and ensuring compliance with all SRC policies and legal obligations. House Manager is the direct supervisor for all Direct Support Professionals or Teams Leads in the program and provides direct service to the people living in the program.

Responsibilities:
- Provide quality care to program participants in an environment that promotes their privacy, dignity, freedom to make decisions, and client satisfaction
- Support daily interactions between participants and staff to promote a culture of care, hope, safety, and empowerment.
- Is a leader & manager to the program staff by demonstrating great accountability, integrity, strong work ethic, and care.
- Provide coaching, feedback, and developmental opportunities to staff members.
- Assist with writing and implementing performance evaluations, warnings, and supervisions.
- Train new staff in all areas including individual plans, personal care requirements and performance expectations.
- Responsible for service planning meetings for members in the program, coordinating with the guardians and case managers around the planning meetings.
- Manage staff to ensure adequate coverage.
- Fully understands and abides by the agency’s code of ethics and all other SRC policies and procedures.
- Fully understands all aspects of participants’ rights and is responsible for promptly reporting any evidence of abuse or violation of members.
- Conduct and facilitate team meetings and ensure that open communication is maintained within DSP staff and address any and all concerns immediately.
- Communicates effectively and respectfully with all members, SRC team members, family members, guardians, case workers, medical providers for member, and other stakeholders.
- Complete all necessary reports and submit to HR department in a timely manner.
- Must maintain medications and ensure adequate supply at all times.
- Approve and make necessary edits to employee timesheets within Payroll system
- Transport individuals, as needed, using personal vehicle (i.e., to appointments, work, etc.)
- Provide crisis intervention during emergency situations.
- Ability to be patient, empathetic, and calm at all times especially in heightened situations
- Manage & oversee documentation for permanent files to ensure compliance with the Department of Health and Human Services’ regulations and SRC standards
- Regularly perform Quality Assurance reviews to ensure documentation is upto SRC standard
- Other duties as assigned

03. Program Manager
Position: Program Manager
Job Type: Full-time
PTO: Accrues at the rate of 1 hr. of time for every 40 hours worked, maxing out at 52 hours annually for sick time and accrues 6.15. hours per bi-weekly pay period for a total of 4 weeks (80 hours) per year.

Overview: Serenity Residential Care provides residential services for adults with intellectual disabilities and autism spectrum disorders. We provide the highest level of care and individualized support to each member (client) who resides in our home-based programs. We are looking for a Program Manager to help us achieve these goals. The Manager reports to a member of the senior management team and partners with management to market services and design, implement, manage, and evaluate person-centered policy, services, and programming.

Specific responsibilities include:
- Overseeing budgets and daily operations for five to ten residential facilities, including supervising House Managers, each of whom have up to ten staff serving members 24/7.
- Develop policies and procedures to ensure quality services for clients and accountability for staff
- Designing, implementing, and managing evidence-based person-centered policy, services, and programming.
- Evaluating effectiveness of services and programming with emphasis on member feedback to achieve continuous improvement.
- Conducting member intake assessments and reviewing case management and health record data to determine service level needs for new members.
- Developing and monitoring Member Function Assessment Support Plans and Behavior Management Plans as appropriate.
- Participating in Individual Support Team (IST) and Positive Centered Plan (PCP) meetings to ensure implementation, review, and necessary modifications of Member Support Plans and Behavior Management Plans.
- Interacting with the Maine DHHS Clinical Review Team (CRT) to negotiate approval of service units as documented in Member Support Plans and Behavior Management Plans, utilizing Kepro software
- Maintaining overall regulatory compliance, including reportable events reporting. Documents and track incidents on Therap’s General Incident Reporting (GER), submits reports to Maine’s DHHS Enterprise Information System (EIS), and completes follow-up reports.
- Networking with the case management, healthcare, and social services community and other professionals to ensure that residential facilities operate at capacity.
- Managing schedules and ensuring good placement of employees for five to ten residential facilities
- Inquiring vender call and filling vacancies and conducting the intake process
- Supervising House Manager and training and coaching them
- Conducting internal investigation in case of any allegation of Abuse, Neglect and Exploitation
- Coordinating and Conducting Monthly House Managers meeting
- Overseeing the management of the buildings and coordinate the fixing any maintenance issue with our Maintenance technician
- Training staff on Positive Centered Plan
- Training Staff on member’s Positive Support Plan

The ideal candidate will have a bachelor’s degree in Social Work and be. :

- Experienced in working with adults with intellectual disabilities and Autism Spectrum Disorders.
- Experienced in and comfortable working in a multicultural environment.
- Familiar with Maine Department of Health and Human Services (DHHS), Office of Aging and Disability Services (OADS), Child Protective Services) (CPS), and Adult Protective Services (ADP) regulations, policies, and IT systems and/or have experience working with or at DHHS/OADS.
- Skilled in case management and developing, monitoring, and evaluating Member Support Plans and Behavior Management Plans.
- Skilled in person-centered service planning with a background in serving adults with intellectual disabilities and Autism Spectrum Disorder, including collaborating with Case Managers, DHHS, OADS, Guardians, and the Primary Care Provider Team (PCP).
- Knowledgeable about MaineCare; Behavioral Support, Modification and Management; Client Rights; ADA; and Basic Protection.
- Outcome oriented, organized, able to manage priorities and balance competing projects, flexible, and work collaboratively with management, staff, other professionals, and members.
- Willing to be on call to respond to emergencies on weekends, holidays, and after hours. This can be a frequent event as some members have critical behavioural needs. Additional qualifications: :

- Valid Maine driver’s license and acceptable driving record.
- Background, criminal history check, and drug test.
- Three references with contact information. References must include a current or previous supervisor and be familiar with candidate’s work history.
